Insurance Reimbursement

All of our physical therapy services are covered under out of network benefits. This is so that we can provide care that YOU need, not that your insurance will “allow.” If you have out of network benefits and would like to submit claims for reimbursement, we are happy to provide what is called a superbill with all of the necessary information to do so. We can also accept FSA and HSA cards.

**Note: Due to regulations outside of our control, we cannot see patients who have Medicare insurance**

Here are a few questions you can ask your insurance company regarding reimbursement for pelvic health physical therapy services:

• Do I have out of network benefits for physical therapy?

• Do I have an out of network deductible?

• Have I met my deductible for the year? If not, how far away am I from meeting the deductible?

• Is there paperwork that must be filled out when submitting the out of network claims? If so, do you provide that paperwork?

• Do I need to be pre-certified or have a physician referral for physical therapy sessions in order to receive reimbursement?
